We found your problem. You shortcode does not look as above but looks as below:

This is because you probably copied link from some editors and that's why it has additional tags.

Please re-write this shortcode manually into theme. Please do not copy it and paste because this is the reason why it does not work in your code!

paste on source should not produce any broken code, and fill the example provided from the editor neither.

This one is ok because is out of special characters coded. Sometimes when you copy code from Visual Editors and other stuff, some additional characters copy also. This is how unfortunately it works and that's why you must be aware of that.
