  for this form we used simple contact form shortcode and also we added below css for styling:

  #contact p, 
  #contact input[type="email"],
  #contact input[type="text"], 
  #contact textarea { color: #fff200; }
  
  #contact input[type="submit"] { color: #000; }
  
  #contact input[type="text"], 
  #contact input[type="email"],
  #contact textarea { background: none !important; border: 1px solid #fff200; }
