Flate Rate shipping not calculating for multiple items

When we have placed a handful of items in the cart the flat rate shipping is not multiplying by the number of items as it should.

Please first disable other plugins and try only with woocommerce enabled.

Everything except for woo commerce is disabled and the issue still exists. In fact, the update cart button is grayed out too.

Update cart is grey because what do you want to update????

Add 1 more item in quantity and look what happens.
