Product options and add to cart button

I'm having three issues - please can you assist:

1) Product options - like a text field - are being displayed NEXT to the add to cart button - instead of ABOVE the art to cart button. (have tied two different product option plugins and behavior is consistent)

2) When the field is a required field - the add to cart button appears to work - gets the successful tick - and the cart slides out from the right - however since the fields are required the product is not added - the error messages are only then displayed in the actual CART page in red at the top.

3) The add GIft voucher in the cart checkout page - throws out the layout of the page instead of simply displaying ABOVE the cart page.
