Can't get contact form to display correctly on mobile

Hi,

I actually submitted this question via the private message a few weeks back but never got a response, so will try here.

On the page http://www.theevolutionway.com/personal-trainer-certificate-in-bangkok/ I can't get the Contact Form to display correctly on mobile (iPhone).

I have tried changing it in lots of ways through the Muffin Builder but can't get it to display correctly.

Please let me know how I can get this working.
