Solved: One page not showing in menu

I posted a couple of pages and place them all in the menu. But, one is not showing in the menu bar. Why is that?

As we can see it's called Secondary Menu. Did you set this menu as main menu under Appearance > Menus > Manage locations section?

Is there a way to place a facebook icon and link on the same line with the menu in the header?

No, it is not possible. Social icons can be put into Action Bar only.

Where does the Action bar show on the front end? And how can I access it and adjust in the control panel?

Action Bat show in the very top of page where phone number and slogan on the left side are as well. But if you mean to control social icons, you can do this from Theme options > Social section.
