items not working properly when going from desktop to phone

I used your theme Contact box and it does not work when I re-size the desktop view to a phone view. The site is not live but can you help ? Screenshots attached.

How can I do that ? The site is not live. How would I send you a link on a mobile device ? You can see by the photos above what happens.
