Button to trigger popup

Hi

On your typography page you have a demo of a button triggering a popup.  I want a button to trigger a popup with a form in it.  Is that possible and if so what would the shortcode be?

That gives a text link not a button?

button="0" means it's text but button="1" will set the button instead.
