Please tell me how to enable the contact form button as on the BeTheme main demo site (http://awesomescreenshot.com/0fc3w8v8fa)

This can be activated in Theme options > Layout > General section and is called "Popup Contact Form Shortcode". Inside field you must put contact form shortcode.

Is there an easy way to change the opacity over the rest of the screen when the contact form has popped out, so that the form stands out more?

Sorry but we do not have any option for that and this can't be done with just some css lines.

Hi there. This option is not in my Theme Options where you specify. I have version 5.0 - is this a later addition?
