Problem to open contact form as popup.

As you can see in https://ferramentasseo.club/melhores-ferramentas-seo when I press the buttom that should open the poup it loads only part of it. Do you know what can I do  to solve that?

The form is complete, it only has 2 fields. Maybe you are implementing the wrong shorcode?
