Missing header on interior pages

The header nav appears fine on the Home page and on all pages when viewed on mobile devices.

BUT! On interior pages, the header disappears on DESKTOP computers. See: https://saddlebackeye.com/am-i-a-candidate-for-lasik-eye-surgery/

Can someone tell me what setting is causing this? Thanks!

So every page except for 1 is now properly showing the nav menu. Please see https://saddlebackeye.com/lasik-finance-options/

Why does it not show the nav menu??

As you can see there is a "loading" class added to the top_bar. This error shows that the user is using a cache plugins of some sort. Please disable all cache plugins and remove browser cache.
Then refresh the page. Avoid JS minification.
