Site Header and Menu Disappear on Blog Page?

Having issues with my header disappearing on the blog section of the site:

main URL: www.myfriendsdentist.com

blog URL: http://www.myfriendsdentist.com/blog/

I am wanting the menu to be consistent on all pages, including the blog.

it does not show because you have selected a blank template for this page.
No header and no footer.
