Contact form doesn´t send an attachment

Hello team:

Please, I need help with this Form (put 4 links/images of this).
The problem is to the user added a file, but this file has not arrived at my email.
And the name and mail also. I don´t know why.

Here I leave 4 links with the images of the configuration of the Form, so they can see if I did something wrong or something is missing, please:

sorry but no images are loaded from those links.

If you have problems sending files from your website, then please contact your hosting provider. All problems related with email services are directly related to the hosting or bad plugin configuration.

Ok, thanks a lot.....in fact the problem solved with the hosting team.
And now, the last question is about how to receive the email, cause the Name and Subject are not received, will be the hosting issue also?

We received the mail, like this: 

From: [your-name] <------This is the issue
Subject: [your-subject] <----- and this!

Message Body:
Por favor, necesito URGENTE contactarme para actualizar mi mail y pagar cuotas.

You need to add a corresponding CF7 Mail tag in the Mail tab for each field you want to use from the Form tab.
