Mailchimp form on the slider

Hello, I bought the theme https://themes.muffingroup.com/be/corporation/ and I can not insert a mailchimp form in the sliders, next to the presentation images at the top of the site.
It's possible? I would like in the slider area, the visitor can fill out the form with email to mailchimp.
How can I do this?

Hi,

I don't think it's possible to attach the "Contact Form" on the slider, but surely you can make a popup which will display the contact form(shortcode).
