Form lost if we update the theme

We are unable to update the theme because we lost one of the customs forms the website has. The form disappears if we update the theme.
Can you please provide some guidance?

Can you tell me what forms are you talking about?
Did you modify the theme files? If yes, then you should use a child theme to not to lose the changes you made.

As far as I know, the theme has not been modified. But not a 100% sure. The form is a customization using "Form Maker" plugin.
