Dropdown order form with button

I am trying to build a dropdown selector like on this page

here you can see that client can select a package with Choose Quantity dropdown (5x, 6x...) from dropdown and then click big green button and be redirected to respective order page, I am trying to build the same thing...

Does anybody know how to build it in betheme??
