Does the Betheme license include access to the Rev. Slider templates?

Templates available in Slider Revolution can be used only with a premium version of this plugin which is not included in the theme package.
However, you can import any slider from our pre-built website. See the following link explaining how to achieve that:

Can you recommend how to add a sign-up feature as in the attached example? I don't want to reinstall another prebuilt template like this one which has the feature. https://themes.muffingroup.com/be/furniturestore/#
