Header and footer not showing up on new page?

I've put the last 5 hours into trying to figure out why header and footer is not showing up on my newly added page...

Hi,
It seems that you have a blank page template enabled for that page.
Edit it, and ensure that it is not selected.
If it is, change it to the Default template.
