I found an example online.

If you could perhaps help me link one of the sub-points, I would be very grateful.

I tried to include this CSS <form></form>, but your system blocks it. Is there another way to show you the script I found? Attached you will find the script as a JPG

I'm sure this isn't your job normally, but I don't know what else to do.
