Duplicate Checkout Fields on Checkout Page

After updating our theme it is showing duplicate checkout fields on the checkout page which is confusing our customers. PLEASE HELP!

    Please always attach a link to your website so we can check it out. If the page is offline(localhost), then our help will be limited. You will have to contact us when the page is online. Also, please make sure that the page is not under maintenance before you provide us with the link.

    It is always a good idea to also attach a screenshot showing your issue.

    Hello,

    The website is https://www.gopetusa.com/checkout/

    Simply add and item to the cart and go to the checkout page and you will see the credit card information has to be entered twice by the customer which is causing confusion.

    I have completely disabled ALL plugins except for WooCommerce and WooCommerce PayPal Payments and the issue still shows up.

    When I switch to the default WordPress Twenty-Three theme the issue goes away and there is no more duplicate fields.

    Please advise.
