Image Upload Adding Captions
When I add an image to my media library, it is automatically adding a new title and caption, as shown in the screenshot below. How do I fix this?

Please, turn off all of the plugins, refresh your cache, and check if the problem persists.
Moreover, if you use a child theme, switch to parent, and recheck it.
