Hi, yes, you missed to install Mail Chimp form before the demo import. So now, or you need to install Mail Chimp plugin and create form manually or you need to clear WP, do complete installation including Mail Chimp form and the import after that.

@resolutesolutions We have no idea what this might be as we did never faced anything similar on our end and we have few different hosting services for tests of course. So we are sure this has completely nothing to do with the theme.
