The popup form appears behind the page content.

The 'Volunteer' button on http://revivalinitiative.org/partner-with-us/ and the 'Give' button on http://revivalinitiative.org/donate/ both have the same issues.

I spoke to the plugin developers and they tried to fix it however they came back with the following:

"Please ask the theme author to have a look and fix the CSS z-index property in the theme."

Please help resolve. Thank you.
