The "Book visit" button you are talking about is from the Header Builder, and you do not use it on your website, that is why clicking on the "test" does not work for you.
The shortcode presented in the article is just an example of possible usage.
So, to summarize, go to Appearance -> Menus and add a menu item with an anchor link. Then go to the page edit and set this ID to a place where you want to link, as I showed in the screenshot above.

Menu for the desktop version of the website: We need darker divider lines in the menu between the pages. Please see the attached. I looked online for this setting but couldn't find anything. I also looked in Theme Options but couldn't find anything there.

I centered the image on the page, but it's not showing that it's centered when pushing "View page." If I change the text on the page, though, it shows the change. But nothing changes with the picture.
Note that I also cleared the object and static server caches.

2nd question: I placed a widget at the bottom of the website and removed it, but there's still a dark gray bar at the bottom of the pages. Please see the attached image. Is there something I can do about that? Or perhaps do I have to add additional CSS? Note that I wouldn't know what CSS to add.
About the image not centering, here are two screenshots, one where the image is centered in the work area (while editing the page), and the other screenshot shows that it's not centered after pushing View page after updating it. Note that you still have access to the back end of the website - logging in with the password (I sent it to you a while back for you to log in).
I have another question. I need to change the text size to the mobile version of the website only (not the desktop version). I don't see those settings in Theme Options. Please see the attached image and notice how small the text size is.

I was finally able to reprocue this issue, and I found out that one of your custom CSS is the root cause.
Go to Appearance -> Customize -> Additional CSS, and remove your CSS one by one to find which is the culprit.

You did not set up font size for the main menu:
When you disable the Auto font size option, you must adjust all font sizes.
